Payments & Fees

All co-pays and non-covered charges are due in full at the time of service. We accept cash, check, PayPal, and all major credit cards. Services may also be covered in full or in part by your health insurance or employee benefit plan.

Insurance

Counselors at Collins Counseling & Associates in Alabama are credentialed with various private insurance companies to provide counseling services. Every health insurance policy is different, but many offer coverage for mental health treatment. Please review your policy or check with your insurance carrier to verify if you have mental health benefits.

What to Ask Your Insurance Provider

  • What is my deductible and has it been met?

  • How many sessions per year does my health insurance cover?

  • What is the coverage amount per therapy session?

  • Is a referral required from my primary care physician?

Cancellation Policy

Please note that there will be a $50 fee for any client who does not show up for a scheduled therapy appointment and has not provided us with notice at least 24 hours in advance.

Patient Forms

We strive to provide all of our clients with a comfortable and convenient counseling experience, whether in person or via our teletherapy services in Alabama. If you’re a new client, please complete the following forms and bring them to your first appointment to streamline your check-in and get right to your counseling session.

*When needed, we are happy to coordinate care with another provider, such as your psychiatrist or primary physician. If you would like for us to partner with your provider, complete this form to authorize the release of your psychotherapy information.

Confidentiality & Privacy Policy

The relationship between a client and a psychotherapist is protected by law. Privileged information about your care cannot be disclosed without your written permission except in certain exclusionary cases.

Exceptions include:

  • Suspected abuse of a child, dependent adult, or senior. We are required by law to report this to the appropriate authorities immediately.

  • Threats of serious bodily harm to another person(s). We are required by law to notify the police and inform the intended victim immediately.

  • If we believe a client intends to commit self-harm or suicide, we will make every effort to enlist their cooperation in ensuring their safety. If a client is uncooperative, we will take further measures as allowed by the law to ensure their safety, with or without their permission.
